EDI Residency Program Forms
Forms on this page are essential to maintaining your equine diagnostic imaging residency program
Note: These forms work best with Chrome or Firefox.
Deadline January 31st of each year:
- Approval/Reapproval/Alternative Residency Program Application Form – Submit this form if seeking approval of a new residency program or undergoing a 3-year residency program reapproval.
- Radiology Residency Training Program Annual Update Form – Required each year other than the year a 3-year residency program reapproval application is submitted to RSEC.
Deadline January 31st and July 31st of each year:
- Semi-Annual Radiology Resident Review Form – EACH resident will need a separate review form. So if you have 3 residents, you will need to submit this form 3 times.
